You can also use your health savings or medical flex spending to cover my services. If you have questions about this, please contact me and we can talk about it.

I will email you a link so that you can register in the client portal and select an appointment in the online schedule that is convenient for you.

Print, complete, and bring with you the Background Information Form: child version or adult version.
